Pull-Through Ureteroneocystostomy for Very Small En Bloc Kidney Transplants from Donors Weighing ≤ 5 kg

Background: Urologic complications are the most dreaded of renal transplantation, particularly when pediatric en bloc kidneys (EBKs) used. Current techniques ureteroneocystostomy (UNC) not applicable to very small ureters kidneys. We reviewed our experience with pull-through in kidney transplantation from donors under or equal 5 kg weight. Material and Methods: The technique was used 32 EBKs. Complications 4-year graft survival discussed. Results: One single thrombosed required nephrectomy. remaining provided good function. Hematuria transient five patients did require fulguration. No leakage experienced. Delayed function occurred 16% cases. primary nonfunction noted. During follow-up period, hydronephrosis and/or pyelonephritis were observed. 95% serum creatinine levels averaging 0.9 mg/dl. Conclusions: procedure proved be safe reproducible.
